In another incident, students described Ms Sullivan as having purple lips, red eyes and slurring her words on November 12, according to reports obtained by NBC News 2.
Students from her claimed she was in a school room closet drinking and took photographs of three wine bottles.
The documents also state when Ms Sullivan was asked by the school why she had three bottles in her purse, she claimed they were from the weekend and she previously had a drink early that same morning.
A school clerk also claimed they tried to help Ms Sullivan with grading that day and said she smelled of alcohol and was acting strangely. Weeks later, she handed in her resignation on December 13.
Ms Sullivan was named the school’s teacher of the year in 2017 and was considered for a prestigious district-wide award.
She has since cited family and healthy issues as the reasons behind her resignation.
